Insurance Certificates

Have a coaching engagement scheduled? Many venues and organizations will request a certificate naming their city/town/arena as additionally insured. This is included in your annual insurance. Your insurance MUST BE CURRENT at the time of the event. If you need to renew please do that HERE before submitting the request for the certificate. Please confirm the exact address and name to be listed on the certificate before submitting your request. This form is for residents of the US and Canada only.
